The Horseback Riding Parental Consent Form serves as the Participant Agreement Release & Assumption of Risk  form for Alaska Horse Adventures LLC. Please review the activity information in the paragraphs below and complete the required information to allow your camper to participate in this spectacular adventure! This form as well as the Medical & Comprehensive Information Form, are necessary to complete your camper's registration for Camp Gan Izzy.
